CAPWN offers a dedicated health program designed to support agricultural workers and their families across our region. This program helps connect eligible individuals to affordable, high-quality care and support services.

Who This Program Is For

This program is available to individuals whose primary employment is in agriculture, including:

  • Farmworkers (seasonal or year-round)
  • Individuals involved in planting, cultivating, harvesting, or processing agricultural products
  • Workers employed in agriculture within the past 24 months

What We Provide

We help coordinate access to a range of healthcare services through our health center, including:

  • Primary care and preventive services
  • Chronic disease management (such as diabetes and hypertension)
  • Women’s health and cancer screenings
  • Pediatric and adult care
  • Dental and vision services
  • Lab work and imaging
  • Immunizations
  • Health education and care coordination

Additional support services may include:

  • Transportation assistance
  • Interpretation services
  • Outreach and patient support

All services are coordinated and authorized through program staff to ensure appropriate coverage.

Eligibility Requirements

To qualify, individuals must:

  • Have a history of employment in agricultural work within the past two years
  • Provide proof of income (30 days)
  • Complete enrollment forms, including sliding fee application
  • Meet income guidelines (generally at or below 200% of the Federal Poverty Level)
