We provide comprehensive diabetes care designed to help you successfully manage your health and prevent complications. Our services include:

• Diabetes self-management education and support, helping you build the skills and confidence to manage your condition
• Personalized education on medications, blood glucose monitoring, insulin use, and healthy lifestyle changes including nutrition and physical activity
• On-site testing and monitoring, including glucose and A1C testing
• Screening for diabetes-related complications, such as high blood pressure, kidney disease, neuropathy, and vascular conditions
• Medication Therapy Management (MTM) services with our on-site pharmacist (by referral from a CAPWN provider), focused on optimizing your medications for effectiveness, safety, and affordability—including access to 340B pricing and patient assistance programs

As a community health center, we accept Medicare, Medicaid, and most major insurance plans.
