Our Primary Care team at Community Action Partnership of Western Nebraska (CAPWN) provides quality, affordable healthcare for patients of all ages – from newborns to seniors. We focus on prevention, wellness, and managing chronic conditions to help you live your healthiest life.

Services Include:

• Annual checkups and wellness exams
• Well-child visits and immunizations
• Care for illness and injuries
• Chronic disease management (diabetes, asthma, high blood pressure, etc.)
• Women’s and men’s health services
• School, sports, and work physicals
• Preventive screenings and lab testing
• Allergy injections under the authority of Allergy Partners of Wyoming

Our compassionate providers take time to listen, answer your questions, and create care plans that fit your needs.

Community Action Health Center offers a range of diagnostic imaging services designed to support timely and accurate patient care. These services help providers evaluate injuries, monitor medical conditions, and assist in our patient’s treatment planning, all while improving patient convenience with on-site imaging and enhancing communication between imaging staff and providers.

Services Include:

• Diagnostic digital x-rays available daily, 9 am to 5 pm. (Special appointments outside of these hours may be available upon patient/provider request.)
• Ultrasound available during multiple days each month.
• Diagnostic Imaging Referral Coordination is available daily as well, through the Referrals Program.
